These forms have been designed to assist the employer and/or apprentice in developing a comprehensive apprenticeship development process and to meet the human resource needs of small to medium-sized businesses without a human resource department.

Employment
Application Form is for employers that want to get more information from
applicants applying for apprenticeships who may or may not have a resume to
attach.
The Performance Evaluation is used to properly assess the apprentices work skills and to assist in giving proper feedback so that the apprentice learns and has a documented form he can refer back to.
Interview Questions have been developed to help the employer hire a good candidate as an apprentice so that both the apprentice and the employer are well matched.
The Apprenticeship Agreement Form is a form that lists the responsibilities of the employer, apprentice and the Industry Training Authority of BC. The employer and apprentice sign this agreement to confirm a commitment to the apprenticeship.

The Return of Service Agreement is a is a form that the employer and apprentice sign confirming that for every year of apprenticeship training there will be portion of the apprentice's time allotted to a return of service when the apprentice has completed their apprenticeship training. This negotiated arrangement is made between the apprentice and the employer to meet the needs of both the employer and the apprentice/journeyperson. This is not a generalized agreement - each agreement will be specific to an apprentice/employer relationship. In order to ensure that your final documents are legally binding, it is recommended that you consult with a lawyer to personalize the agreement to your business situation - what is provided here is intended only for use as a guideline.
The Apprenticeship Aptitude Test can be used by the apprentice to determine his/her educational abilities or for the employer to determine if the apprentice will be able to handle the theoretical component of the apprentice training in a certified training institution. The test has been designed to be used only as a tool to assist with upgrading and is not intended as a recommendation to pass or fail an apprenticeship candidate. The Apprenticeship Record Book is a group of documents that Employers, Apprentices and Industry Partners have requested to record and store apprenticeship and on-the-job training information in a comprehensive format.
The web-based Apprenticeship Record Book was developed in partnership with Northern Lights College and is based on the Northern Lights Trades Worker Log Book © 2003, that was developed by Jeff Lekstrom, Manager of Trades and Apprenticeships, NLC.
A link to the ITA Registration Form will direct apprentices and journeypersons/employers to the official provincial form that is necessary to indenture/register an apprenticeship. The apprentice and employer are both registered with the Industry Training Authority of BC and given separate registration numbers.
